As of January 1st, 2019, all accreditation/certification applications must be submitted through our McMaster CHSE site.

New Online CHSE Accreditation Application

As of January 1, 2019, all accreditation/certification applications must be submitted through our McMaster CHSE Online Accreditation Application System. An application checklist and all required forms can be found online on the CHSE website.

Effective January 1, 2019, please refer to the new fee structure for Accreditation Review.
