Online Accreditation Application

As of January 1, 2019, all accreditation/certification applications must be submitted through our online system. A non-refundable fee is charged for application reviews. Applications will not proceed for review until payment has been received. Please refer to our fee structure for Accreditation Review.

CPD Office Required Forms

Please ensure you complete these forms using the provided version:

  • Declaration and Approvals Form
  • Disclosure of Conflict of Interest Form
  • EDNA Single Session – Educational Design and Needs Assessment Mapping Form (Use for activities with a single session on a dedicated topic with a single presenter/panel of presenters)
  • EDNA Multi Session – Educational Design and Needs Assessment Mapping Form (Use for activities with multiple sessions/topics and presenters)
  • Sponsorship List
  • Recurring Activity Supplement (Use only for activities with fixed content that are recurring and applying for re-accreditation)
