The prevalence of chronic kidney disease in Canada is increasing. To ensure patients at high risk of progressing to end-stage kidney disease are seen in a timely fashion, the Ontario Renal Network developed the Kidneywise toolkit to determine which patients with chronic kidney disease (CKD) need to be evaluated by a nephrologist. This has resulted in primary care providers having to provide CKD care for patients who are early in the disease trajectory or who are at low risk of progression. This continuing medical education event has been designed to provide primary care providers with the knowledge and tools to be able to provide evidence-based renal care to low-risk patients, know when to refer to nephrology and support patients with advanced kidney disease on their illness journey in collaboration with their primary nephrologist.
Target Audience
Family Physicians, Specialists, Internists, Hospitalists, Physician Assistants, Nurse Practitioners, Registered Nurses, Residents, Students, and Other Health Professionals.
Learning Objectives
By attending this activity, participants will be able to:
- Learn how to advance health equity within kidney disease.
- Apply an approach to the identification, diagnosis, and management of chronic kidney disease.
- Identify resources available to primary care providers for assisting in the identification and management of chronic kidney disease.
- Understand when and how to use new and emerging therapies in the management of chronic kidney disease.
- Determine which patients require nephrology referral and which can be managed by their primary care provider and be able to counsel their patients in this regard.
- Develop a basic understanding of renal transplantation and an approach to post-transplantation primary care.
- Develop an approach to dietary counselling for patients with chronic kidney disease.
- Apply an approach to the diagnosis and management of hyponatremia and hyperkalemia in the outpatient setting.
